Admissions/Discharges

Admitting/Utilization Management Department
This department is involved with Patients' Choice Medical Center patients in many aspects of their care.  Each patient begins with this department through registration and many patients are assisted by this department upon discharge.

Outpatient/Inpatient Registration
Outpatient and Inpatient registration is handled by admitting clerks.  They are responsible for collecting patient data, which includes patient demographics and insurance information.  They direct the patient and/or family to the appropriate area in the hospital depending on which service they need. 

Emergency Room (ER) Registration
ER patients are registered in an area just outside the Emergency Department.  The clerks in this area also collect pertinant patient information.  They assist the ER staff with patient care by submitting patient orders and directing the flow of traffic in that area.

Case Management
Case Management involves many things.  It involves assessing, planning and facilitating option and services to meet patient and family health needs.  This is accomplished through communication and available resouces to promote optimal quality and cost effective outcomes.

Discharge Planning
Discharge Planning involves assessment of patients' needs for discharge.  The process is started at the beginning of their Hospital stay.  Depending on the patient/family needs and requests they may be referred to any of the following:

            Home Health
            Skilled Care
            Hospice
            Help with Sitters
            DME Referrals

Each patient and/or family member is involved in the choice of referral and agency they wish to assist in their care after discharge.
